We’ll confirm the monthly fee, appointment allowances and current terms, then help you complete the application.

A £10 joining fee is payable at the practice. Please read both pages of your agreement before signing and return your completed form to reception.

Does the plan cover every treatment?
No. The plan covers the listed appointments and benefits. Other treatment may be charged separately; we’ll explain costs before treatment.
Which plan should I choose?
Start with the examination and hygiene frequency recommended for you. We can explain the differences and help you choose.
Can I pay for visits individually instead?
Yes. You can view our private treatment fees and pay for appointments individually.
